An MRI is an interpretation. Needle arthroscopy is a direct look. Using the Arthrex NanoScope®, I can see cartilage, ligaments, and joint surfaces in real time, often in the office, under local anesthesia which means treatment decisions are based on what I’ve actually seen, not an inference from imaging. I’ve used this technology as my standard arthroscopic platform since 2019, across more than 700 procedures.
Needle arthroscopy isn’t a replacement for traditional arthroscopic or open surgery when that’s genuinely what’s needed it’s a more precise, less invasive first step that, in appropriately selected cases, can serve as both the diagnosis and the treatment in a single visit
NanoScope Needle Arthroscopy
The Arthrex NanoScope® uses a needle-sized camera to visualize a joint through a tiny portal, without the setup, recovery, or facility requirements of traditional arthroscopy. I adopted this technology early in 2019 and have used it as my standard diagnostic and, in select cases, therapeutic arthroscopic platform ever since.
Knee, Shoulder, and Elbow Procedures
Needle arthroscopy is applicable across the major joints I treat knee, shoulder, and elbow for both diagnostic evaluation and, in appropriately selected cases, direct treatment of what’s found. The specific findings and treatment options vary by joint, but the underlying approach is the same: see it directly, then decide.
Diagnostic and Operative Applications
In many cases, needle arthroscopy serves a purely diagnostic purpose confirming or ruling out a suspected finding before committing to a larger procedure. In appropriately selected cases, it’s also operative: certain meniscus, cartilage, and loose-body procedures can be performed through the same needle-sized portal, combining diagnosis and treatment into a single visit.
Meniscus and Cartilage Treatment
For select meniscus tears and cartilage lesions, needle arthroscopy allows direct treatment including meniscectomy under local anesthesia in appropriately selected cases without the recovery burden of a traditional operating-room procedure.
Synovectomy
Loose-Body Removal
Loose fragments of cartilage or bone within a joint a common source of catching, locking, or persistent pain can often be identified and removed through needle arthroscopy, avoiding a larger procedure for a problem that doesn’t require one.
Athletic Injuries
For athletes with an acute joint injury, needle arthroscopy offers a fast path to an accurate diagnosis often within the same visit which can meaningfully shorten the time to an appropriate treatment plan and reduce time lost to a wrong initial diagnosis based on imaging alone.
In-Office, ASC, and Hospital Settings
Needle arthroscopy is flexible in where it’s performed many diagnostic and select therapeutic procedures are done in-office under local anesthesia, while more involved cases may be performed in an ambulatory surgery center or hospital setting. The setting is matched to what the specific procedure requires, not a one-size-fits-all default.
Potential Benefits in Appropriately Selected Cases
Combination With Surgical Biologic Augmentation
Needle arthroscopy findings often inform whether biologic augmentation PRP or BMAC is added to a treatment plan, either at the time of the procedure or as a following step. Seeing the tissue directly is often what determines whether a biologic approach makes sense for a given joint.
